To place an order please follow these steps:
1) Click on "Order Form" below (pops up new window for Adobe Acrobat).
2) Fill out the Shipping Information at the top.
2) Fill out the quantity of books in the left hand column (a running subtotal will be calculated automatically).
3) Press the "Calculate" button at the bottom of the screen to calculate shipping and handling.
4) Please review the new Total amounts in the right column.
5) Print the form and mail your request with payment to the address shown above.
Shipping and handling charges will be calculated automatically. A description of the charges can be found here.
International orders: please send a check payable in US dollars and drawn
against a bank doing business in the U.S.
